California Department of Education is in the process of formally translating SCOHR reporting forms into other languages. While this process is completed, the Office of Oral Health encourages LOHPs to use the Interim Spanish language versions of these forms, translated by San Mateo County (SMC). Note: LOHPs will need to customize the forms with their county information.
