Each school is required to track, tally, and report specific data from the KOHA forms for each child eligible for KOHA. The forms are collected and tracked continuously throughout the school year.
- If your school hosts on-site screenings: Completed forms from the screening day are given to school staff by the dental screening provider/organization.
- The data is reported into a database called the System for California Oral Health Reporting (SCOHR) at the end of the school year or by July 1st at the latest.
Data Collection & Inputting KOHA Data Into SCOHR
In SCOHR, you can input your data by signing into SCOHR and choosing from two options:
- Enter aggregate data for one school at a time using the Data Input Form. You can improve your data collection when entering data into SCOHR this way by using the Excel spreadsheet on the right. The video at the bottom of this page explains how to use the Excel spreadsheet for Data Input Form entry.
- Upload aggregate data for multiple schools at a time using the bulk upload function
If the Excel spreadsheet above is not used, an alternate data entry aide is the PDF fillable form to the right, where aggregate totals can be entered for the school. It color codes data line items, and maps them on the second page to where they can be found on the actual KOHA forms.
Tip: Try to identify a KOHA champion at the school, school district, county office of education, and/or LOHP to track and input the KOHA data into SCOHR.

KOHA Visuals and Tables
The auto-calculation Excel spreadsheet is a tool you can use to create tables and visuals by adding your summary report KOHA data.
Note: This template is designed for reports generated starting in the 2022-23 school year (after the changes in SCOHR to align with the new KOHA forms).
In SCOHR, go to "Reports" and select "Summary Report." Please note: Your SCOHR account may provide other types of reports. This template is only designed for the report labeled "Summary Report" in SCOHR.
