Data

Each school is required to track, tally, and report specific data from the KOHA forms for each child eligible for KOHA. The forms are collected and tracked continuously throughout the school year. 

  • If your school hosts on-site screenings: Completed forms from the screening day are given to school staff by the dental screening provider/organization. 
  • The data is reported into a database called the System for California Oral Health Reporting (SCOHR) at the end of the school year, or by July 1st at the latest 

Data Collection & Inputting KOHA Data into SCOHR

When you start to screen Kindergarteners, you can improve your data collection by using an Excelsheet to track (samples on the right) . 

In SCOHR, you can input your data by signing into a SCOHR account and choosing from two options:

  1. Enter aggregate data for one school at a time using the Data Input Form 
  2. Upload aggregate data for multiple school at a time using the bulk upload function

Tips: Try to idenitfy a KOHA champion at the school, school district, county office of education, and/or LOHP to track and input the KOHA data into SCOHR.
