Until the end of the school year or May 31st, whichever date comes first, send at least three reminders to parents/caregivers who have yet returned KOHA forms.

- If your school hosts on-site screenings: prior to the oral health screening day, a letter is usually sent to parents/caregivers informing them of the details of the screening day. It should also provide instructions about how to opt-out of the screening or include the On-Site Screening Opt-Out Letter.
